Becoming a Client

Eligibility 
Park House is open to adults (18 years of age or above) diagnosed with HIV/AIDS who need a supportive environment or are diagnosed with a mental illness.

Becoming a Client
Persons interested in attending Park House can contact us, their case manager or physician to schedule a tour and complete an application. Learn more about our programs and services.

Payment
Services may be reimbursable by medical assistance or private insurance. Financial assistance may be available through Abbott Northwestern Hospital.
