Patient and family programs

Anoka

Our health improvement programs help patients, families, and community members manage their risk for heart disease, diabetes and stroke.

Health screening, exercise programs, education sessions and support groups are available on a variety of topics.

Focused Asthma Management Education (FAME)

Respiratory services offers individualized education and instruction on the tips and tools that can help you manage your asthma. A respiratory care practitioner will provide you with information on getting the most from your breathing medications and help you to identify the potential triggers and troublemakers that cause asthma attacks. A baseline breathing test is performed. Ask your doctor to refer you for this valuable disease self-management program.

Cardiac education and support

Unity's Health Improvement Center offers classes and support groups for people living with or wanting to prevent heart disease. For more information, call the Health Improvement Center 763-236-3535.

Tobacco intervention program

The tobacco intervention program offers an inpatient and outpatient component. If you are at Unity Hospital for medical reasons and are a smoker, you are given the opportunity to speak with a tobacco cessation counselor. The counselor will assist you in developing a plan to quit tobacco products usage and follow up with you after discharge.

If you are interested in outpatient programs, call Allina Class Registration at 612-262-3333.
